Really, the question is just how bad was Joe's brain problems that required two surgeries? And just how much damage did it due before the surgery and after the surgery?
From what we see now, along with his dementia, it appears that the surgeons took too much damaged material leaving a large gap in the process, but then decided that from Joe's history in government employment, no one would notice any change in his behavior!
And they were right!
